AO 213 Vendor Information/Certification (1/2012 | 2 pages | fillable PDF)
Effective January 30, 2012, the Administrative Office of the U.S. Courts has issued a new form, the AO 213, for establishing new vendors in the Judiciary's accounting system. The single form includes all the information required to comply with Judiciary, IRS, and Congressional requirements. The IRS Form W-9 Request for Taxpayer Identification Number and Judiciary Provision 3-5 Taxpayer Identification and Other Offerer Information are no longer required.
